As our Eugene and Roseburg practices grow, we’re seeking a compassionate Nurse Practitioner or Physician Assistant Certified Provider with experience in women’s health and hormone replacement therapy (HRT) to expand access and better serve our communities. The selected candidate will help determine which location to fill.

Responsibilities and Duties:
- Provide direct patient care through office visits, telehealth, and same-day appointments. This is an in-clinic position, not remote.
- Perform comprehensive health assessments, including detailed histories and lab reviews, to identify root causes of symptoms.
- Develop and adjust individualized care plans incorporating lifestyle medicine, nutrition, and medically guided weight loss strategies.
- Provide women’s health services with a focus on hormone replacement therapy (HRT), menopause management, and reproductive health.
- Monitor patient progress closely, adjusting treatment plans and therapies as needed for sustained results.
- Educate and empower patients in lifestyle changes, hormone health, and preventative wellness practices.
- Collaborate closely with Monarch’s multidisciplinary team and community partners to ensure coordinated, high-quality care.
- Maintain compliance with clinical, regulatory, and documentation standards.
- Document visits in the EHR (Practice Fusion/AdvancedMD) in a timely and compliant manner.
- Participate in team meetings, quality improvement initiatives, and training sessions.
- Support clinic operations by contributing to a positive, team-oriented environment.
Required Qualifications:
- Active, unencumbered, Oregon license as a Nurse Practitioner or PA-C.
- Board certification.
- Minimum 1 year of clinical experience preferred (functional/integrative medicine experience a plus)
- Strong interest in preventive, functional, and lifestyle medicine.
- Excellent communication and patient relationship skills.
- Comfortable working in a collaborative environment
- Proficient with electronic health records (Practice Fusion/AdvancedMD experience a plus)
- Familiarity with CPT coding for billing procedures; ICD-10 coding knowledge required for accurate documentation of diagnoses.
- Ability to work collaboratively within multidisciplinary teams.
- Critical thinking skills with the ability to maintain high standards of patient safety and confidentiality per HIPAA guidelines.
