This is a full-time, 32–40 hour per week position offering competitive base pay up to $150,000 plus $35,000 in annual bonus opportunity and comprehensive benefits. Your Responsibilities
- Conduct comprehensive medical and functional assessments of patients in skilled nursing and rehabilitation facilities.
- Collaborate with an interdisciplinary team—including physicians, physical therapists, and occupational therapists—to develop and monitor individualized care plans.
- Manage and evaluate patient progress, addressing medical barriers to recovery and optimizing outcomes.
- Participate in daily clinical rounds and coordinate with nursing staff and facility administrators to ensure quality care delivery.
- Document visits and care plans using the Doctor Now EHR, integrated with PointClickCare.
- Provide ongoing communication with the primary care team, contributing to chronic care management and readmission prevention programs.
- Maintain communication with regional clinical supervisors to ensure clinical excellence and team collaboration.
Qualifications
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Massachusetts.
- Strong interpersonal, communication, and clinical assessment skills.
- Experience or interest in Physical Medicine & Rehabilitation (PM&R) preferred.
- Ability to work independently while collaborating with a multidisciplinary team.
