Job Description:
We are seeking a dedicated and self-motivated Advanced Practice Provider to join our team in a Nephrology office and dialysis facility. The position involves collaborating with experienced Nephrology providers in the care of patients with chronic kidney disease and those undergoing dialysis. The ideal candidate will work in a supportive environment where supervision from a Nephrology provider is always available.
Responsibilities:
- Provide direct patient care in a dialysis setting.
- Conduct patient assessments, including reviewing medical history and performing physical exams.
- Assist in the management of patients with chronic kidney disease and those on dialysis.
- Order and interpret diagnostic tests.
- Educate patients about treatment plans and medication management.
- Collaborate with a multidisciplinary team to ensure high-quality patient care.
Requirements:
- Active VA Medical license and DEA certification required.
- Experience in Nephrology is preferred but not required.
- Must be a team player and self-motivated with the ability to work independently.
- Strong communication and interpersonal skills.
