Position Title: Nurse Practitioner (NP) / Physician Assistant (PA) – Family Practice
Location: Florence Family Practice | Florence, Arizona
Schedule: Four 10-Hour Days
- Monday, Tuesday, Thursday: Florence
- Friday: Coolidge
Hours: 8:00 AM – 7:00 PM
Setting: Outpatient Family Medicine
Travel: Required one day per week (to Coolidge clinic)

Position Overview
We are seeking a skilled Nurse Practitioner or Physician Assistant to join our community-based family medicine clinics in Florence and Coolidge, AZ. This full-time role provides a consistent, flexible four-day schedule and the opportunity to deliver full-spectrum primary care to patients of all ages in a supportive, mission-driven practice.

Key Responsibilities
- Perform comprehensive physical exams and health assessments
- Diagnose and manage acute illnesses and chronic conditions
- Order and interpret labs, imaging, and diagnostic studies
- Prescribe medications and implement treatment plans
- Educate patients on disease prevention, treatment adherence, and lifestyle modification
- Conduct minor in-office procedures (e.g., wound care, injections, suturing, immunizations)
- Document all patient encounters accurately in the EHR
- Coordinate referrals and support continuity of care
- Collaborate with physicians, RNs, and allied staff
- Participate in quality improvement initiatives, case reviews, and care planning
Qualifications
- Active and unrestricted Arizona license as an NP or PA
- Graduate of an accredited NP or PA program
- Master’s degree in Health Sciences or related field
- For NPs: Approval by the Joint Board of Medical Examiners and Board of Nursing and active Arizona RN license
- Minimum 2 years of experience in outpatient family medicine or primary care
- EHR experience preferred
