Overview
We are seeking a dedicated and compassionate Wound Care Provider to join our team. The ideal candidate will have a diverse skill set and experience in various healthcare settings, providing exceptional care to patients of all ages. This role requires a commitment to improving the quality of life for those in our care, whether in a hospital, assisted living facility, or home care environment.
Duties
- Provide direct patient care, including assistance with daily living activities and personal hygiene.
- Administer medications and treatments as prescribed by healthcare professionals.
- Conduct patient assessments and monitor vital signs to ensure optimal health.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Maintain accurate medical documentation and records in compliance with HIPAA regulations.
- Assist with diagnostic evaluations and procedures such as IV infusion, catheterization, and specimen collection.
- Support patients with chronic conditions through education on disease management and health coaching.
- Facilitate communication between patients, families, and healthcare providers regarding care needs and progress.
Skills
Candidates should possess a combination of the following skills:
- Proficiency in EMR/EHR systems such as Epic, Cerner, or eClinicalWorks.
- Experience in critical care settings, including ICU or emergency medicine.
- Knowledge of medical terminology, CPT coding, ICD-9/ICD-10 coding, and documentation standards.
- Familiarity with procedures related to gastroenterology, dialysis, pain management, and behavioral health.
- Competence in performing physical examinations, venipuncture, phlebotomy, and administering injections.
- Experience working with diverse populations including pediatrics, geriatrics, and individuals with developmental disabilities.
- Understanding of infection control practices and aseptic techniques.
- Ability to provide compassionate end-of-life care in hospice and palliative settings.

Base Pay: $100,000.00 - $145,000.00 per year
