Job Overview:
We are seeking a dedicated and skilled Nurse Practitioner (NP) to join our vascular team. In this role, you will collaborate closely with vascular surgeons to deliver seamless care plans for patients. Key Responsibilities:
- Provide direct patient care at outreach clinics in multiple locations, assessing and managing vascular conditions.
- Work closely with our vascular surgeons to create and coordinate care plans for specific patients.
- Serve as a liaison between patients, surgeons, and outreach clinics to ensure cohesive communication and care delivery.
- Coordinate follow-up appointments, procedures, and patient education.
- Learn and apply the fundamentals of vascular surgery through provided training.
- Maintain detailed and accurate patient records.
- Collaborate with the healthcare team to improve patient outcomes and care efficiency.
Qualifications:
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA-C) with an active state license.
- Prior experience in a clinical setting (experience in vascular surgery or related fields is a plus, but not required as training will be provided).
- Strong organizational and communication skills.
- Willingness to travel to multiple clinic locations.
- Ability to work collaboratively in a multidisciplinary team.
- Compassionate and patient-centered approach to care.
