About the Role
As a part-time NP or PA, you’ll deliver comprehensive care to a high-need, high-risk geriatric population. We’re looking for someone with a background in Internal Medicine, Family Medicine, or Geriatrics who can work autonomously while collaborating with our multidisciplinary team. This role combines hands-on patient care with opportunities for remote support, like prescription refills, lab reviews, and patient communication, giving you a balanced and flexible schedule.
Key Responsibilities
- Conduct thorough physical exams and wellness assessments for seniors
- Follow up on care plans to ensure the best possible outcomes
- Propose and manage treatment plans for chronic and acute conditions
- Maintain accurate patient records and manage appointments effectively
- Educate patients on preventative care, treatments, and healthy lifestyle choices
- Engage in both fieldwork and remote care, offering prescription refills, lab reviews, and patient outreach
- Collaborate with a supportive team dedicated to holistic care
Qualifications
- Completion of an accredited NP or PA program
- Experience in geriatrics, internal medicine, or family practice
- Strong communication, active listening, and analytical skills
- Compassionate and committed to providing patient-centered care
- Motivated to serve a geriatric population with complex care needs
